Accident Report Form (Document template)
Should an accident occur during your shoot it is important that you log the details of what happened as well as the action that were carried out. You will need to get the person/s involved to witness the report (either on the spot or at the soonest possible safe opportunity - if the person/s involved cannot witness the report and confirm that is an accurate account of the events you should get it witnessed by a third party). The accident report form can then be used to prove your candour of care as well as for any H&S risk prevention in the future.
You will also need to log which medical treatments (if any) were offered and administered as well as the medical professionals that carried out the treatments.
